What is the Self-Assessment Questionnaire?

What is this questionnaire for?

The self-assessment questionnaire presents some questions on the conditions of your trip and/or stay abroad.

Based on your answers, a risk score is automatically calculated. This score makes it possible to assess the degree of risk taken during your trip and/or stay abroad:

  • If you return from a red zone and get a low score: you did not take any risk during your trip and/or your stay abroad, you do not need to go into quarantine, nor get tested for Covid-19.
  • If not, You come back from a red zone and must be tested and respect a quarantaine of 7 days. You will receive a code (a combination of 16 digits and letters) allowing to request a Covid-19 test at a sampling location.
  • If you are coming back from an orange or green zone and have a high score you took risks during your trip. It is particularly important to limit your social contacts, to respect hygiene measures and to wear a mask for 14 days after your arrival in Belgium. During this time it is also important to monitor your symptoms and contact your doctor if you have a fever or other symptoms of COVID-19, , mentioning your travel history.

How to properly complete the questionnaire?

In order to obtain the most accurate assessment, it is very important that you answer the questions honestly.

What happens to the data completed in the questionnaire?

Your answers will only be used to calculate a general risk score and will not be saved.

If you are prescribed a test, only the data necessary for sample collection (national register number and contact details) will be used.
